The official UB-04 Data Specifications Manual, 2017 Edition is the source of UB-04 billing information adopted by the National Uniform Billing Committee (NUBC). This Manual, available by annual subscription license, contains the updated specifications for the data elements (codes) included on the UB-04 (uniform bill) claim form.

• The online acceptance of a User License Agreement is required prior to completing the order transaction.
• The Manual is a PDF eBook using the browser-based Lizard Safeguard Secure PDF Viewer for viewing. There is no software required to view the eBook.
• You will receive a confirmation email within one hour of your online order from aha-orders@pbd.com with "eContent Product Delivery Notification from American Hospital Association" in the subject line.
• You will also receive an email from webmaster@e3-iguana.com with the subject line “Important Information Regarding Your Recent AHA Purchase.” The email from webmaster@e3-iguana.com contains access instructions and your individual log-in credentials. Make sure to check your SPAM folder if email hasn't arrived. If you do not receive the email within one hour of your online order, please email ub04@healthforum.com.
• All subscribers are on the same subscription cycle of July 1 to June 30; there are no partial or pro-rata subscriptions. The 2017 Manual eBook will be available for your use through June 30, 2017, the end of the licensing period. At that time, the eBook will expire and you will need to purchase a new license for the 2018 Edition of the Manual.
• UB-04 Manual content cannot be shared, copied, modified or transformed into derivative works. One copy may be printed for archival purposes.

• Complete the order form. We must have an email address on the Order Form. This email address is where the eBook link will be sent and must be the user of the eBook. Single User UB-04 Manual cannot be forwarded to other users.

Acceptance of this License Agreement is required to fulfill all UB-04 orders placed by mail, fax or email. Sorry, orders using credit cards as payment may not be emailed
